What a healthcare SEO company costs and what it must know
A healthcare SEO company works inside constraints that most search agencies never meet. The client is frequently a covered entity under federal privacy rules, the content makes claims that regulators expect to be substantiated, the tracking code sits on pages where patients describe symptoms and book appointments, and the search engine itself applies extra scrutiny to pages that could affect a reader's health. Any one of those can turn an ordinary marketing decision into a compliance incident. This page sets out what the specialism actually involves, what moves the retainer, and how to compare proposals from firms that will all describe themselves as healthcare experts.

How to buy healthcare search work safely
- Ask about the business associate agreement first. If the agency will handle patient lists, recorded calls, form submissions or anything else that identifies a patient, it needs a signed agreement covering that handling. Ask in the first email. A firm that does not recognise the question has not thought about what its tracking code collects either.
- Inventory the tracking before anything is installed. List every tag, pixel, chat widget, call recorder and heatmap tool the agency proposes, what each collects, and where it sends it. Then decide which pages they may run on. Booking, symptom and portal pages are the ones to protect, and the default there should be off.
- Name a clinical reviewer for every content claim. Pages describing conditions, treatments or outcomes need a qualified person to check them before publication, and their name and credentials should appear on the page. Agree who that reviewer is and how much time they can give each month before the content plan is sized.
- Define the outcome the report will show. Traffic is not the goal for a practice with a full book. Agree whether success is new patient appointments, referrals, service line specific enquiries or something else, and require the monthly report to lead with that figure rather than sessions and rankings.
What moves the retainer
Scale moves it most. A single specialist practice competing in one metro is a narrow programme. A multi site group with dozens of locations, several service lines and a physician directory is a different order of work, because every location and every clinician needs accurate, consistent, structured details and the directory itself becomes a technical project. Service line breadth moves it next: each line has its own competitors, its own research and its own content calendar, so a proposal covering six lines for the price of one is either thin or aspirational.
Compliance overhead is the factor buyers underestimate. Clinical review cycles, legal sign off, privacy review of tracking, and the extra care needed around testimonials and patient stories all add elapsed time and cost that consumer agencies do not carry. The federal rule at 45 CFR 164.508 sets out when a patient authorisation is required for marketing use of protected health information and what that authorisation must contain, and building a compliant process for obtaining it is part of the engagement, not an afterthought.
Content, claims and credibility
Google's guidance on creating helpful, reliable, people first content asks whether content is produced primarily for readers, whether it demonstrates first hand expertise, and whether it is the kind of page a reader would trust or share. Health topics are where that guidance bites hardest, which is why named, credentialed authors, dated reviews, cited clinical sources and a clear editorial policy are worth more here than in any other vertical. An agency proposing bulk produced condition pages with no clinician attached is proposing exactly the pattern that guidance describes unfavourably.
Claims are the other exposure. Advertising has to be truthful and substantiated before publication, and the Federal Trade Commission's health products compliance guidance sets out the standard of evidence expected for health related claims. In practice that rules out outcome guarantees, cure language and testimonials implying typical results without support. It also governs reviews: the FTC has adopted a rule addressing fake and manipulated reviews and testimonials, so any agency offering to supply or incentivise patient reviews is offering a legal risk rather than a service.
How to compare proposals
Build one table: minimum term, monthly fee, named monthly deliverables, clinical and legal review hours required from your side, tracking to be installed and where, and what you own at the end. The review hours column and the tracking column are the two that separate healthcare specialists from generalists, and they are almost never in the sales deck.
Then apply the standard filters. Nobody can guarantee a search position. Google states that it does not evaluate or endorse third party services, so certification badges are marketing rather than evidence. Ask for two healthcare clients you may contact and ask each specifically what the agency did about privacy, tracking and clinical review. That question, asked of a reference, sorts this market faster than any portfolio.
Questions people actually ask
- Does a marketing agency need a business associate agreement?
- If it creates, receives, maintains or transmits protected health information on your behalf, yes. That can include patient lists, call recordings and identifiable form data. Raise it before any tooling is installed rather than after the first incident.
- Can we run analytics and advertising pixels on booking pages?
- Do so with great care. Information transmitted from pages where patients book or describe symptoms may be protected, so the safe default is to keep third party marketing tags off those pages or configure them to transmit nothing identifying, with the decision documented.
- Can we publish patient testimonials?
- Only with a valid written authorisation meeting the federal requirements for marketing use, and the testimonial must reflect the patient's genuine experience with any material connection disclosed. A signed treatment consent form does not cover marketing publication.
- How long before a healthcare programme produces patients?
- Local visibility for a single practice can move within a few months. Competitive service lines in large metros take longer, and clinical review cycles lengthen everything. Agree leading indicators at ninety days so the programme is judged on evidence rather than impatience.
